Employee Exit Checklist

Whether an employee leaves voluntarily or involuntarily, it's easy to overlook important steps in the exiting process. Click here to access a customizable Excel checklist (also below) to assist your practice with the process.


Property Return

  • Obtain name badge                     
  • Keys (office, desk, locker, etc.)
  • Cell phone
  • Computers (modems, software, accessories, etc.)
  • Company vehicles
  • Credit cards
  • Parking access
  • Company manuals/files/financial information
  • Instruments/tools
  • Uniforms (anything with company logo)

Human Resources

  • Verify contact information is current
  • PTO/Vacation Payout:     YES     NO      Amount/Hours: __________     
  • Final pay period processed
  • Submit termination to payroll
  • Submit termination to 401K
  • 401K eligibility/rollover
  • Health insurance coverage termination
  • COBRA enrollment
  • Complete exit interview
  • Building and/or suite access – list locations
  • Obtain office/building key(s)
  • Number of keys
  •  Obtain building badge or delete card
  • Surgery center/hospital ID cards, keys and parking cards

IT Access Removal

  • Windows
  • Email
  • Practice management system
  • Electronic medical record
  • Drug inventory management
  • Copay assist portals
  • Other internal communications (i.e.: Pingboard)
  • Surgery center/hospital EHR access terminated

Communication

  • Send email to physicians and staff
  • Send email to key vendors
  • Document employee file
  • Ensure signed copies of Confidentiality/Non-Compete/Non-Disclosure/Intellectual Property agreements
  • Remove employee's name from directories and website
